Patients who would like to request for home visits by the Hospice At-Home Programme nurse, need to obtain a referral letter from the hospital or family physician. The referral letter should contain relevant information of the patient's medical condition which is important for care of patients. If you do not have a doctor attending to you, you may contact the nurse coordinator who will visit you and advise you accordingly.

NOTE:
Any patient who has advanced cancer and has been told by his/her doctor that cure is unlikely is eligible for admission to the Hospice At-Home Programme.
